Investigations of the launch failure of Japan's M5 space booster are focusing on the nozzle. Fragments of the carbon graphite lining of the nozzle were found on the launch pad, indicating that the lining may have failed immediately upon launch resulting in a burn-through (and failure) 25 seconds later.--Stephen V Cole
